The mobile app is the key component in engaging with patients / clients. It serves as the interface for them to see what tasks they have to complete in a care plan.

The mobile app allows patients / clients to:

  1. Review the tasks and care team members added to a task
  2. Complete tasks that are assigned to them, such as taking a health measurement using a device or entered manually, or booking an appointment with a referee provider
  3. Access documents uploaded for patients, such as within a task
  4. Review progress through a useful progress tab

Review Tasks

Before patients / clients begin actioning a task, they must accept tasks that have been assigned to them

Complete Tasks

Once the care team and tasks are reviewed, the patient / client can complete the task. There are 6 task types:

  • Appointment bookings
  • Take a Test, which is for completing a laboratory/pathology test, or an assessment
  • Health measurements (track progress)
  • Medications
  • Prescribed treatments, such as exercises and monitoring/treatment of wounds at home
  • Read/View Documents, such as reading educational materials or YouTube videos

Patients / Clients can also view a summary of how they are doing across all their tasks. This is a useful screen to assure them that they are on track, or if they need some assistance from their primary carer.

Health Measurements (Track Progress)

Patients / Clients have a powerful feature to record health measurements. This feature allows the provider to more effectively treat them using detailed data.

They can enter the data manually, or have them updated automatically through Bluetooth connected devices, such as glucometers, connected weighing scales, and step counting from activity monitors.

Treatments

Patients can also complete treatments at home that you assign them, such as exercises and routines.

Read / View Documents

Documents that are added to the patient’s / client’s care plan can be easily accessed through the mobile app.

Medications

Patients / Clients can also receive reminders about medications prescribed to them.
