Although Nomadic Care focuses on patients completing tasks, many care plans require actions to be completed by another provider. These tasks often involve completing and returning a form.
This is where the Form feature comes in.
It allows the care plan coordinator to assign a task — either Book a Referral or Take a Test — to another provider, requiring them to complete and submit a form.
There are two task types where another provider can be assigned to complete a form:
- Book a Referral
- Take a Test
Book a Referral #
1. In the selected care plan condition within the patient record, click “Book Appointment.”
2. Choose “Referral” as the appointment type.
3. Toggle the “Form” switch to the ON position.
This converts the task into a form-filling request for a nominated referee provider.
You must also set the due date for the form.
For example, you may request completion within 10 days. You can optionally schedule a follow-up appointment for when the form is due.
4. Enter the referee’s details and click “Save.”
The referee will receive an email notification outlining the task. If they are not already a Nomadic Care user, they will also receive an invitation to register.
There is also a “Patient Task” toggle.
Enabling this adds a task for the patient to book the appointment with the referee. This is optional and is OFF by default when the Form toggle is enabled.
5. Once assigned, the task will appear in the referee’s dashboard.
An illuminated document icon in the Actions column indicates that a form requires completion.
6. The referee can click the document icon to download, complete, and upload the form back into Nomadic Care.
7. Once submitted, the completed form is stored in the patient’s Documents tab, alongside all other related documents.
That’s it. Nomadic Care manages the reminders and follow-ups automatically, helping ensure important forms and assessments are not missed.
Take a Test #
The Take a Test task is used when requesting a specific assessment, such as a laboratory, pathology, or clinical test.
The workflow is similar to Book a Referral:
- When creating the task, toggle the “Form” switch ON.
- Additional referee fields will appear and must be completed.
- The nominated referee will be required to complete and submit the attached form.
As with Book a Referral, you can enable the “Patient Task” toggle if you want the patient to book the appointment with the provider.
