Step 1- Compose care plan with tasks for patient to complete through Nomadic Care Provider Portal
Through the easy-to-use provider portal, doctors and carers create a care plan for a patient. A care plan is a list of tasks with goals and dates to complete them by: booking follow-ups and referrals, health measurements, exercises, medications, reading materials and video tutorials, and more. These tasks help patients stay on top of tasks, and collect valuable information from them – including health measurements entered by the patient or from devices connected to their phone.
Nomadic Care can also suggest tasks based on previously completed tasks by other patients and from medical literature, offering a powerful knowledge base. Doctors ultimately decide on which tasks are included.
Doctors and carers can also setup a telehealth session through a video conference, and write private health notes during the consultation.
Patients sign up with a care plan code and then review tasks. They have ultimate control of which tasks they do - approving them when first logging in.
Tasks that are due soon are shown on the top, making it super easy for patients to stay on top of their health. Some tasks require health measurements to be taken, which are usually automatically collected from the mobile phone via Google Fit or Apple Healthkit. Nomadic Care takes the stress out of taking regular measurements – and giving doctors more data to make better decisions.

Often patients will not report back to a doctor or carer until their health suffers or they experience symptoms. Through regular health measurements, doctors can be alerted through Nomadic Care’s Patient Progress monitoring tool if a patient is in need of extra support. Nomadic Care encourages both health providers and patients to be proactive with healthcare – ensuring the best outcome for the person at the centre of care, the patient.
